Contact us and let us know that you and your family have chosen to support the Foundation in this way. We are committed to meeting your wishes for these special gifts and will send notification cards to the family member of your choice when gifts are made. We’d be happy to provide a list of donors and their addresses to facilitate the writing of thank you cards.

Consider setting up a memorial page
If you are interested in receiving a personalized link to share with your friends and family wishing to make a memorial gift, visit UHNfoundation.ca/fundraise and build a permanent tribute page.

Inform your family and friends of your wishes
Your friends and family can make gifts in memory of your loved one by visiting our website or your personalized web link, or by calling our office. The best way to inform your friends and family of your wishes is by sharing the suggested wording below in an obituary notice or via other means.

“In lieu of flowers, donations to the {insert program or department name if applicable}, at UHN Foundation would be appreciated by the family. Please visit UHNfoundation.ca/tribute or call 416-603-5300.”

Additionally, we can assist by providing printed donation cards and prepaid return envelopes to the funeral home or service venue of choice.

We will make a commitment to keep the family informed of all gifts made as well as answer any questions they may have. If your organization does not have generic donation cards, we would be happy to provide Foundation donation cards and prepaid return envelopes for the service.
